Public forms vs. Patient Portal (no PHI on public forms)
- Public forms on this site are for basic inquiries only (name, phone, email, short message).
- Please do not submit PHI (medical details, diagnoses, results, account numbers) through public forms or standard email.
- Use the Patient Portal for messages, refills, results, and visit-related information. Clinicians and referral partners should send PHI by secure portal or fax.
Starting services, home visits, and scheduling
- Online requests help us start the process but are not confirmed until you hear from us. We may adjust or reschedule based on team availability and safety.
- Telehealth or phone support may be used for certain check-ins; your clinician will advise when an in-person visit is needed.
- To change or cancel a visit, follow your confirmation instructions or call (480) 943-1220. Missed-visit policies may apply and are shared at scheduling.
Referrals and coordination
- Hospitals, clinics, home-health agencies, and families may refer. We aim to confirm within one business day and a care coordinator will follow up.
- Helpful details include: recent discharge date, primary diagnosis or reason for referral, current medications, ordered therapies, and preferred contact.
Insurance and billing (high-level)
We work with many plans. Coverage varies by plan and service. Before your visit, confirm benefits, copays, coinsurance, and deductibles with your insurer. Any cost estimates are informational and not a guarantee of coverage or final charges.
