Please do not include medical details in public forms. Use the Patient Portal for anything clinical.

• Approximate location: derived from IP address.
• Cookies and similar tools: to remember preferences and understand site use.
• Analytics: we may use tools such as Google Analytics 4 (GA4) to measure traffic and improve the site.

From service providers and partners

• Vendors that help with scheduling, hosting, email delivery, analytics, security, and forms may process information for us.
• If you use the Patient Portal or view embedded maps/videos, those providers may collect data under their own policies.

We do not use public website forms to collect or manage PHI. Clinical care is handled through the Patient Portal and our clinical systems.

We are based in Arizona. While Arizona does not currently have a broad consumer privacy law like some other states, we choose to honor reasonable requests from Arizona residents to access, correct, or delete personal information collected through this website (subject to legal limits and exemptions).
